Steady support for older adults and people with disabilities

Individual and family social services from one suite in Coeur d'Alene. Terri K Johnson works with older adults, people with disabilities, and the families who plan beside them.

How we show up

Promises we keep in the room with you

The person stays in the plan

Support is written around the adult who receives it. A family conversation does not replace that person's own say.

Families hear the same account

Caregivers get the same facts we use in the suite. No separate story for the home and the office.

The path

From the first phone call to a weekly plan

Four plain steps. We do not start support before the person and family have agreed what it covers.

2

Use the suite or a remote slot

Meet at Prairie Avenue, by phone, or by video.

3

Name the support

Older-adult help, disability supports, or a family plan.

4

Keep a cadence

A weekly rhythm you can see, not a vague check-in.

The office

Care rooted on West Prairie Avenue

Chrysalis, Inc. is an individual and family social services practice for older adults and people with disabilities. The work happens from Suite 3, not from a chain of offices.

Coeur d'Alene is the home base. If a visit needs to be by phone or video, that is arranged from the same desk.

  • Services for the elderly and persons with disabilities
  • Family planning conversations with the person present
  • One Idaho location, opened in 2023
Supports

Support for daily life, not a rotating roster

Three lines of work. The longer description of each one is on the supports page.

Older adult support

Help with the shape of a week: routines, check-ins, and a clear line back to family.

Disability supports

Community time and daily tasks done with the person, at their pace.

Family social services

A shared plan so caregivers and the person are working from one account.
